I wish to participate in water sports activities including, but not limited to Aqua Park Inflatable Obstacle Course and Cable Park activities including all activities organised by Sheffield Cable Water Ski Ltd (referred to as "the Activities").

I am 18 years old or older.

I confirm that I have read, understood and can meet the Entry Requirements for the Aqua Park and/or the entry requirements for the Cable Park as defined in the Code for Safe Skiing (depending upon which activity you are taking part in). These documents are available in the Reception area and on the website https://www.sheffieldcablewaterski.com/entry-conditions-rules-aqua-park/.

I agree that I have read, understood and accept the Rules of the Aqua Park available by clicking this link: https://www.sheffieldcablewaterski.com/ and/ or the Code of Practice for Safe Skiing (depending upon which activity you are taking part in) and I will undertake the Activities in accordance with these rules.

I accept and acknowledge that I am responsible for my possessions and my own safety whilst taking part in the Activities.

I agree that I will only carry out moves and tricks and use features that are within my skill level and of which I am able to retain control of at all times and those which are within the rules of the particular activity. Moves and tricks and use of features are performed entirely at the participant’s own risk and will not hold Sheffield Cable Water Ski Ltd liable for any injury they may attain whilst taking part in these activities.

I certify that I can swim competently (at least 25 metres to partake in the Aqua Park activity and 50 metres for the Cable activity) and to the best of my knowledge I do not have a medical condition which might have the effect of making it more likely that I will be involved in an accident which could result in injury to myself or others. To the best of my knowledge I am not pregnant.

APPLICABLE TO THOSE SIGNING FOR UNDER 18 YEAR OLDS (MINORS)

I am the parent/guardian of the child/ren listed above who is/are under 18 years of age. I wish that/those child(ren) to participate in water sports activities including Inflatable Aqua Park and Cable Park activities including all activities contained within (referred to as "the Activities") organised by Sheffield Cable Water Ski Ltd.

I agree that I am responsible for the child(ren) in my care and undertake to ensure that he/she/they undertake the Activities in accordance with the Rules of the Aqua Park for the Aqua Park and the Code for Safe Skiing for the Cable Park along with any other advice given prior to or during the session.

I declare that if I am not the parent or guardian of the child/ren I have authority from the child/ren's parent or guardian to sign this form and disclose the child(ren's) personal information.

I acknowledge that I am responsible for the safety and supervision of the child(ren) named above (and the safety of our possessions). I will ensure that I pay particular attention to any 7-9 year olds on the Aqua Park activity where Sheffield Cable Water Ski has a supervision policy.

I agree that I will inform my children to only carry out moves and tricks and use features that are within their skill level and of which they are able to retain control of at all times and those which are within the rules of the particular activity. Moves and tricks and use of features are performed entirely at the participant’s own risk and will not hold Sheffield Cable Water Ski Ltd liable for any injury they may attain whilst taking part in these activities.

I certify that the child/ren meet the minimum advertised age requirements for the activity, can swim competently (at least 25 metres for the Aqua Park activity, 50 metres for the Cable activity) and to the best of my knowledge do not have any medical conditions (including pregnancy) which might have the effect of making it more likely that he/she/they be involved in an accident which could result in injury to him/ her / themselves or others (if in any doubt, please check with the child/ren's parents or guardian).

APPLICABLE TO ALL

I acknowledge and accept that the Activities require a reasonable level of fitness and can be physically demanding, and I/he/she /they should not take part in the Activities unless I/he/she/they am(is/are) physically able to. I acknowledge and accept that the Activities are dangerous and there is a risk of personal injury when undertaking these Activities.

In the unlikely event of an accident, or loss, damage to any personal effects Sheffield Cable Water Ski LTD accept no liability for any direct or indirect loss, damage or injury arising from or in connection with the Activities (except for death or personal injury caused by Sheffield Cable Water Ski LTD’s negligence)

I acknowledge and accept that CCTV images of all participants at the site will be monitored and recorded in the interest of preventing crime and public safety.

I acknowledge that I and my child(ren) (if applicable) have been provided with the Entry Requirements of the Aqua Park and the Safety Rules of the Aqua Park to participate in the Aqua Park activity and/ or we/I have been provided with the Code of Practice for Safe Skiing if participating in the Cable Park activity and that I have already read and fully understand the above.

HAVE READ THE CODE OF PRACTICE, AND ACCEPT THE RISKS ASSOCIATED WITH THE NATURE OF THE ACTIVITY.

I UNDERSTAND THAT WATER SKIING IS A HAZARDOUS SPORT.

I DO NOT KNOWINGLY SUFFER FROM EPILEPSY OR ANY OTHER MEDICAL CONDITION RENDERING ME UNFIT TO SKI.

I CAN SWIM 50 METRES UNAIDED.

CAN SEE 50 METRES WITHOUT THE AID OF SPECTACLES.

CODE OF PRACTICE

WEARING A BUOYANCY AID/ IMPACT JACKET AND HELMET IS COMPULSORY.

TAKE AVOIDING ACTION IF YOU SEE SOMEONE IN THE WATER.
IF IN DOUBT, LET GO OF THE TOW ROPE IMMEDIATELY.

IF YOU SEE ANY HAZARDS IN THE WATER - SKIS, DEBRIS, OR A SKIER IN TROUBLE, GO TO THE NEAREST BANK AND REPORT TO A MEMBER OF THE CABLE STAFF IMMEDIATELY.

IF YOU FALL, LET GO OF THE HANDLE! TRY TO AVOID FALLING FORWARDS -
SIT DOWN, OR IF FALLING SIDEWAYS TRY TO CURL INTO A BALL. IF YOU NEED
ASSISTANCE, RAISE YOUR ARMS ABOVE YOUR HEAD, OTHERWISE SWIM QUICKLY TO THE NEAREST SHORE THEN WALK BACK TO THE START, CARRYING YOUR EQUIPMENT WITH YOU.

IF YOU ARE IN THE WATER AND YOU SEE A FREE HANDLE COMING TOWARDS YOU, MOVE OUT OF THE PATH OF THE HANDLE, OR IF THERE ISN’T TIME, DUCK UNDER THE WATER.

ALWAYS OBEY ALL INSTRUCTION GIVEN TO YOU BY CABLE SKI STAFF.

NEVER SKI/RIDE INTO THE STARTING AREA OR SHALLOW WATER.

NEVER DELIBERATELY SKI/RIDE TOO CLOSE TO A PERSON IN THE WATER.

NEVER WRAP THE TOW ROPE AROUND ANY PART OF THE BODY, OR PLACE ANY PART OF THE BODY THROUGH THE TOW HANDLE.

ANY HORSEPLAY ON OR OFF THE WATER, USE OF ALCOHOL OR DRUGS, OR
DISOBEYING INSTRUCTIONS GIVEN BY ANY CABLE SKI STAFF WILL RESULT IN YOU BEING ASKED TO LEAVE THE SITE IMMEDIATELY, AND WITHOUT REFUND, AND MAY RESULT IN YOU BEING BANNED FOR AN INDEFINITE PERIOD.

HELMETS MUST BE WORN AT ALL TIMES

ONLY DESIGNATED HIRE BOARDS TO BE USED ON THE CABLE SKI FEATURES

ALL JEWELLERY MUST BE REMOVED

ONLY SKI/WAKEBOARD TO WITHIN YOUR OWN ABILITY

NO SWIMMING INTO THE AQUA PARK DESIGNATED AREA
